I hereby certify that my pet(s) is/are in good health and have not been ill with any communicable condition in the last 30 days. I also certify that I have read, understand and agree to the following:

I understand that I am responsible for any harm or damage caused by my pet(s) while they are in daycare or boarding with Pets West Aledo, LLC. I shall indemnify and hold harmless Pets West Aledo, LLC, as a result of my failure to inform Pets West Aledo, LLC of any pre-existing conditions of my pet(s) (e.g., aggressive behavior toward humans or other pets).

I understand and agree that Pets West Aledo, LLC will not be liable for damage or injury caused by my pet provided that reasonable care and precautions were taken by Pets West Aledo, LLC employees.

I understand that daycare and boarding is a place where my pet(s) will be around other pets and my pet(s) may be placed in a crate or kennel known as “timeout” due to unruly behavior as determined by Pets West Aledo, LLC employees, not to exceed thirty minutes. If for any reason your pet is injured during daycare or boarding but the injury is not serious, as determined by Pets West, LLC employees, you may or may not be notified, unless you communicate otherwise. If the injury is serious, as determined by Pets West Aledo, LLC employees, you will be immediately notified and if you are not available, Pets West Aledo, LLC employees will take appropriate action to protect your pet(s) including but not limited to, scheduling an examination by a local licensed veterinarian. I understand that in dealing with pets there are inherent risks of injury or illness. Such risks include but are not limited to injuries resulting from rough play and kennel cough. I understand that when pets play together, nicks, scrapes, and scratches may occur. Pets West Aledo, LLC will do everything possible to protect your pet(s) from harm.

If a daycare dog is not picked up by closing time, they will be fed our house food and charged one night of boarding in addition to the daycare fee.

I understand that if my pet is not picked up by the contract time period, Pets West Aledo, LLC employees will continue to provide food and care for up to three (3) days after the contracted pickup date and time. If you have still not contacted Pets West Aledo, LLC employees with a plan to pick up your pet(s), then Pets West Aledo, LLC will consider your pet(s) to have been abandoned and necessary steps will be taken to turn your pet(s) over to the proper authorities. You authorize Pets West Aledo, LLC to charge your credit card on file for all time and expenses incurred during daycare or boarding.

I have read and understand the policies and conditions and know the location, hours, rules, and charges of daycare and boarding at Pets West Aledo, LLC. If my pets are not picked up by the contracted pick-up time, then additional charges can or will be applied at the contracted daycare or boarding rates.

I will provide a copy of my pet(s) shot record(s) which must be current and up to date for all essential and medically recommended vaccinations from a licensed veterinarian. Your pet(s) vaccination records must be provided in either hard copy or emailed directly from your veterinarian to boarding@petswestaledo.com prior to daycare or boarding. Failure to provide proper documentation of your pet(s) vaccinations will be grounds to deny service.
